      Medicare / Medicaid Information



Pathways HomeCare Center is a provider for both Medicare and Medicaid.  If you are outside the Denver metro area, please contact us to determine if we can provide in your area and still bill your insurance.  The process for obtaining equipment funded by either of these payer sources from Pathways HomeCare Center is as follows:
  1. Acquire a physician's prescription for the equipment with the patient's name, date, and diagnosis
     
  2. Submit the RX with insurance card(s) and patient demographics (name, address, phone, sex, DOB, SS#, height, weight, next of kin) information.  This can be faxed to 303-365-0111
Medicare's coverage is 80% if it's allowable.  The remaining 20% will be billed to any secondary insurance, but may drop the beneficiary.  Pathways HomeCare Center will bill a variety of products assigned, which means that Medicare is billed directly.  Other items are billed non-assigned, which means the equipment is paid for up front by the customer and Pathways HomeCare Center submits a claim to Medicare as a courtesy bill on the customer's behalf.
Medicaid requires prior authorization for most equipment.  Pathways HomeCare Center must submit the proper paperwork and obtain approval before dispensing the equipment.
